Required forms Insurance - Primary
coverage is the responsibility of each camper and his
parent(s)/guardian(s). KSU has purchased secondary insurance for each
participant that will cover over and above the participant's primary
coverage. You will need to download and print off the medical release form return it by the first day of camp. KSU Medical Release Form - Download Here
