2008 Newsletter & Camp Checklist:
Welcome to the Indiana Diving Academy! I’m looking forward to working with you this summer. Please take some time to read this form and fill out all appropriate paper work to be submitted for diving camp. If you have any questions regarding forms or the camp in general, please do not hesitate to contact us. I look forward to a very successful 2008 summer dive camp.

U.S. Diving Registration:
All Indiana Diving Academy campers must be registered with U.S. Diving for insurance purposes. Limited lesson membership is $15.00 and provides four months of coverage from the time of enrollment from US Diving. We are no longer being covered by the AAU Insurance for our summer camp, you MUST sign up for U.S. Diving!

Medical Consent Form:
In order to enable the Health Center of Indiana University and/or other health facilities in Bloomington to provide prompt care to your minor son or daughter, we urge you to read and complete this consent form. Please return it promptly to the program sponsor. In this way, we can help your child without delay should an emergency occur.

Release of Liability Agreement:
In order to participate at the Indiana Diving Academy, a Parental Consent and Release of Liability Agreement must be signed and on file. Please download the agreement and return to Indiana Diving Academy.

Diving Safety Form:
Attention Divers and Parents. U.S. Diving (Gabriel, 1990) recommends that all U.S. Diving coaches provide divers and parents with a warning of the potential dangers inherent within the sport of diving.

Current Diving List & New Dives To Learn:
For you to get the most out of the Indiana Diving Academy, we would like to know more about your current diving list, experience, competition level, and dives you would like to learn prior to your coming to camp. Please tell us all the dives you can currently complete for each level. Complete the Current Diving List & New Dives To Learn form, download here. If you run out of room, please write them on the back of the page.
